Job Summary:
Provide specialized physical therapy services to patients in a skilled nursing facility, focusing on restoring function, reducing pain, and improving quality of life. Collaborate with the healthcare team to develop and implement individualized treatment plans.

Responsibilities:

• Assess patients' physical conditions and determine appropriate therapy interventions.
• Develop and modify treatment plans based on patient progress and needs.
• Document patient evaluations, treatment sessions, and outcomes accurately.
• Educate patients and caregivers on exercises and techniques to promote recovery.
• Collaborate with physicians and other healthcare providers to optimize patient care.
• Ensure compliance with regulatory and facility policies regarding patient treatment.

Qualifications:

• Valid State Physical Therapist license.
• Proven experience working in a skilled nursing or long-term care setting.
• Strong understanding of rehabilitation techniques and modalities.
• Excellent communication and patient engagement skills.
• Ability to work effectively within a multidisciplinary team.
• BLS certification preferred.
