Job Summary:

Registered Nurse providing patient-centered care by assessing, planning, implementing, and evaluating individualized plans of care in collaboration with the multidisciplinary team. Delivers clinical judgment in recognizing and reporting changes in patient condition while supporting clear patient education and accurate documentation.

Responsibilities:

• Assess patients to identify mental, physical, and spiritual needs.
• Plan, implement, and evaluate individualized care plans in collaboration with the care team.
• Provide clear explanations and instructions to patients and responsible persons.
• Monitor patient condition and recognize, document, and promptly report changes using nursing clinical judgment.
• Support performance of core measure responsibilities related to patient care.
• Maintain complete and accurate medical records and participate in continuing education.

Qualifications:

• Hold a current Registered Nurse license in Illinois.
• Maintain current CPR/BLS certification.
• Maintain current ACLS certification.
• Demonstrate strong nursing clinical judgment for deterioration recognition and escalation.
• Communicate professionally to manage conflict and support a healthy workplace environment.
• Demonstrate competency with ICU/stepdown and peri-procedural patient care workflows as applicable.
