Job Summary:

The Physical Therapist provides skilled physical therapy services to patients in a skilled nursing setting, focusing on restoring function, improving mobility, and enhancing quality of life. The role includes evaluating patient needs, developing individualized treatment plans, and coordinating care to support safe discharge outcomes.

Responsibilities:

• Assess patients’ mobility, strength, balance, pain, and functional limitations through standardized evaluations.
• Develop and implement individualized treatment plans based on clinical findings and goals.
• Deliver therapeutic interventions to improve ambulation, transfers, gait training, and functional independence.
• Document patient progress, treatment responses, and plan modifications in accordance with clinical standards.
• Collaborate with nursing and interdisciplinary teams to ensure coordinated, goal-directed care.
• Educate patients and caregivers on exercises, assistive devices, and safety strategies.

Qualifications:

• Hold a valid Physical Therapist license in the applicable state.
• Possess a Doctor of Physical Therapy (DPT) degree or an equivalent accredited physical therapy qualification.
• Demonstrate strong clinical skills in functional mobility, gait training, and therapeutic exercise.
• Exhibit proficiency with treatment documentation and adherence to clinical documentation requirements.
• Show experience delivering therapy in post-acute or skilled nursing environments (preferred).
• Demonstrate effective communication and patient-centered care in interdisciplinary settings.
