Job Summary:
The Physical Therapist provides patient-centered rehabilitative services in collaboration with a multidisciplinary team to enhance mobility and functional independence.

Responsibilities:

• Evaluate patients of all ages to develop individualized treatment plans.
• Implement therapeutic interventions including manual therapy, gait training, and neuromuscular re-education.
• Document assessments, treatments, and progress accurately within designated timeframes in electronic medical records.
• Educate patients and caregivers on injury prevention, home exercises, and safety measures.
• Assess patient needs for assistive devices and train patients in their proper use.
• Collaborate with healthcare team members to coordinate care and optimize outcomes.

Qualifications:

• Graduate from an accredited physical therapy program.
• Hold a valid New Jersey Physical Therapy License.
• Possess 2+ years of experience for job code 3602 or 0–2 years for job code 3600.
• Demonstrate strong clinical skills in musculoskeletal, neurological, and post-operative rehabilitation.
• Exhibit excellent communication skills tailored to diverse patient populations.
