Job Summary:

Seeking a dedicated Physical Therapist for home health services, providing patient care through home visits and collaborating with a multidisciplinary team to ensure quality rehabilitation.

Responsibilities:

• Evaluate patient's physical condition and develop personalized treatment plans.
• Deliver therapy sessions at patient homes, ensuring documentation and progress tracking.
• Maintain reliable transportation to navigate up to 75 miles daily for patient visits.
• Communicate regularly with referral sources and team members regarding patient progress.
• Complete all documentation accurately within 24 hours of each session.
• Comply with all agency policies, safety standards, and regulatory requirements.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Possess a minimum of 1 year full-time adult home health experience.
• Be proficient in HomeCare Homebase EMR system.
• Have current CPR and BLS certifications.
• Demonstrate comfortable travel within the designated territory.
• Be able to pass a Tier 2 travel compliance check if applicable.
