Job Summary:
Provide outpatient physical therapy services to patients, assessing and developing treatment plans to improve mobility and function. Ensure compassionate care while maintaining compliance with facility protocols and licensing requirements.

Responsibilities:

• Evaluate patients' condition and establish personalized treatment plans.
• Implement therapeutic interventions to promote recovery and improve patient outcomes.
• Document patient progress accurately in electronic health records.
• Educate patients and families on exercises and injury prevention strategies.
• Collaborate with healthcare teams to coordinate patient care and share relevant information.
• Maintain up-to-date licensure and certifications, adhering to compliance standards.

Qualifications:

• Hold an active SC or Multi-State Compact Physical Therapist License.
• Possess at least 1 year of outpatient therapy experience.
• Demonstrate proficiency in Epic charting system and outpatient rehab protocols.
• Be BLS certified.
• Capable of floating within scope to meet facility needs.
• Willing to work day shifts, 8:00 am – 4:30 pm.
