Job Description
Job Summary:
Seeking an experienced RN Case Manager to coordinate care and facilitate patient transitions within an acute hospital setting. The role involves collaborating with multidisciplinary teams to ensure optimal patient outcomes and resource utilization.
Responsibilities:
- Conduct initial and ongoing patient assessments, reviewing care plans and gathering comprehensive psychosocial, medical, and financial information.
- Develop and implement cost-effective, personalized care transition plans in collaboration with patients, families, physicians, and the healthcare team.
- Monitor patient progress and ensure adherence to level of care (LOC) requirements through documentation and coordination with physicians and medical records.
- Facilitate timely discharge planning, including addressing barriers, coordinating post-acute care, and ensuring regulatory compliance.
- Communicate effectively with internal and external stakeholders, including social services, payers, and community resources, to support patient needs.
- Participate actively in interdisciplinary team rounds and process improvement initiatives to enhance care quality and efficiency.
Qualifications:
- Valid California RN license with current Nursys verification uploaded within the last two weeks.
- Minimum of 5 years of RN experience, including inpatient acute care case management preferred.
- BLS certification required; additional certifications such as AHA or ARC are advantageous.
- Proven ability to lead care coordination efforts and collaborate with multidisciplinary teams.
- Strong knowledge of CMS and Joint Commission transitions of care regulations and documentation standards.
- Excellent communication, critical thinking, and interpersonal skills to expertly navigate diverse patient and team interactions.
