Job Summary:
Provide specialized physical therapy services to patients in a skilled nursing facility setting. Assess, develop, and implement treatment plans to promote patient mobility and functional independence.

Responsibilities:

• Evaluate patients' physical conditions and develop personalized treatment plans.
• Administer therapeutic exercises, manual therapy, and modalities to improve patient function.
• Monitor patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on exercises and techniques to maintain progress.
• Document patient assessments, treatments, and outcomes accurately.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Demonstrate clinical expertise in acute and long-term care settings.
• Possess excellent communication and patient interaction skills.
• Be experienced in developing and modifying treatment plans.
• Maintain current CPR and first aid certifications.
