Job Summary:
The Physical Therapist provides rehabilitative services to patients in a Skilled Nursing Facility, focusing on restoring mobility and improving quality of life. They work closely with healthcare teams to develop and implement personalized treatment plans.

Responsibilities:

• Evaluate patients’ physical conditions and develop tailored treatment protocols.
• Administer therapeutic exercises and manual therapy techniques to promote recovery.
• Monitor and document patient progress, adjusting treatment plans as needed.
• Educate patients and families on proper exercise techniques and post-treatment care.
• Collaborate with multidisciplinary teams to provide comprehensive patient care.
• Ensure compliance with all healthcare regulations and facility policies.

Qualifications:

• Possess current state licensure as a Physical Therapist.
• Hold a degree in Physical Therapy from an accredited program.
• Demonstrate excellent communication and interpersonal skills.
• Maintain CPR and BLS certifications.
• Have prior experience in SNF or related settings preferred.
• Able to work independently and as part of a team.
