Job Summary:

The Acute IP Physical Therapist provides inpatient rehabilitation services for patients in a hospital-based setting, delivering evidence-based therapy to support functional recovery and discharge readiness. This role collaborates with an interdisciplinary team to assess needs, develop treatment plans, and deliver skilled interventions using established clinical pathways.

Responsibilities:

• Evaluate patients and perform physical therapy assessments to establish functional baselines and therapy priorities.
• Develop individualized treatment plans and set measurable goals in coordination with the care team.
• Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Document patient progress and treatment outcomes in the electronic medical record (Epic).
• Coordinate with nursing and interdisciplinary staff to support safe patient transitions and discharge planning.
• Provide education to patients and families on home exercise programs and safe mobility techniques.

Qualifications:

• Hold a SC or Physical Therapist license eligible under the Multi State Compact at the start of the contract.
• Possess at least 1 year of acute inpatient physical therapy experience.
• Maintain current BLS certification.
• Demonstrate strong clinical reasoning and the ability to manage medically complex cases.
• Communicate effectively with an interdisciplinary team and patients to support care continuity.
