Job Summary:
Provide outpatient physical therapy services to patients, focusing on rehabilitation and recovery. Collaborate with clinical teams to develop and implement personalized treatment plans to improve patient outcomes.

Responsibilities:

• Evaluate patients' physical conditions and establish appropriate treatment goals.
• Develop and execute individualized therapy plans based on patient needs.
• Document patient progress accurately within the Epic charting system.
• Assist in float coverage across similar units or sister facilities as needed.
• Maintain compliance with facility policies, licensing, and certification requirements.
• Adhere to scheduled hours and communicate any scheduling conflicts proactively.

Qualifications:

• Hold an active South Carolina or Multi-State Compact Physical Therapist License.
• Possess at least one year of outpatient therapy experience.
• Obtain and maintain BLS certification.
• Demonstrate proficiency in using Epic for clinical documentation.
• Be flexible to work 5x8-hour day shifts from 8:00 am to 4:30 pm.
• Ability to float within scope and work in a hospital outpatient rehab setting.
