Job Description
Job Summary:
Registered Nurse provides comprehensive nursing assessment and coordinates individualized hospice care using the nursing process. The role develops care plans, manages case assignments, and collaborates with an interdisciplinary team to deliver safe, quality patient-centered care in accordance with applicable regulations and agency policies.
Responsibilities:
- Conduct comprehensive assessments of patient and caregiver needs to guide quality hospice care.
- Develop and communicate realistic plans of care and ensure caregivers are included in the planning process.
- Participate in interdisciplinary team meetings and complete reassessments within established timeframes.
- Implement medical plan of care and nursing interventions, ensuring infection control and safe, effective service delivery.
- Serve as case manager by managing caseloads and coordinating visit timing per the interdisciplinary plan.
- Delegate tasks in accordance with LPN scope of practice and ensure documentation, visit notes, and recommendations are completed and acted upon.
Qualifications:
- Hold an active Registered Nurse license in accordance with state requirements.
- Demonstrate competency in hospice nursing, symptom management, and pain control.
- Show experience with IV use and maintenance and complex wound care, including wound VAC when required.
- Provide evidence of strong clinical assessment, care planning, and evaluation skills.
- Maintain current knowledge of regulatory requirements, documentation standards, and agency protocols.
- Demonstrate the ability to educate patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions.
