Job Summary:
Provide specialized hospice nursing care to patients in their homes, ensuring comfort, symptom management, and emotional support. Deliver personalized, compassionate care while collaborating with interdisciplinary teams to enhance patient quality of life.

Responsibilities:

• Administer comprehensive end-of-life nursing care, addressing physical, emotional, and psychological needs.
• Manage pain and symptoms effectively, utilizing evidence-based practices to ensure patient comfort.
• Case manage patients, coordinating care plans, and ensuring proper documentation through Oasis experience.
• Collaborate with physicians, therapists, and community providers to deliver holistic patient care.
• Educate patients and families on disease progression, care strategies, and emotional support resources.
• Monitor patient progress and adjust care plans as needed to maintain optimal comfort levels.

Qualifications:

• Demonstrate Oasis experience, especially in home health settings, with proficiency in case management.
• Hold a valid Registered Nurse (RN) license in the appropriate state.
• Require home health or hospice nursing experience, with a focus on terminal and palliative care.
• Possess strong assessment, communication, and interpersonal skills.
• Ability to work independently in patient homes with minimal supervision.
• Proven ability to handle emotionally challenging situations with professionalism and compassion.
