Job Summary:
Provides comprehensive occupational therapy services to patients in a Skilled Nursing Facility setting. Assesses patients' needs, develops treatment plans, and implements interventions to promote independence and improve functional abilities.

Responsibilities:

• Evaluate patients' physical, emotional, and cognitive abilities to determine appropriate therapy plans.
• Develop individualized treatment plans addressing patients' specific needs and goals.
• Implement therapeutic interventions to enhance coordination, strength, and mobility.
• Monitor patient progress and adjust treatment strategies accordingly.
• Document clinical assessments, treatment plans, and progress notes accurately.
• Collaborate with healthcare team members to coordinate care and ensure optimal patient outcomes.

Qualifications:

• Hold a valid Occupational Therapist license or registration in the state of practice.
• Possess a degree in Occupational Therapy from an accredited program.
• Demonstrate proficiency in clinical assessments and therapeutic techniques.
• Have experience working with elderly populations in a rehabilitation or SNF setting preferred.
• Strong communication and interpersonal skills.
