Job Summary:
Providing skilled physical therapy services in a home health setting within a 60-mile radius. The therapist will evaluate, develop, and implement treatment plans to promote patient recovery and independence.

Responsibilities:

• Conduct comprehensive physical therapy assessments of patients in their homes.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Document patient progress and maintain accurate records using the Meditech electronic medical record system.
• Collaborate with team members and coordinate care effectively across different departments.
• Educate patients and caregivers on exercises, injury prevention, and treatment adherence.
• Travel between patient locations, adhering to mileage reimbursement policies and scheduled hours.

Qualifications:

• Hold a valid Missouri Physical Therapist license with possible PT compact privileges.
• Possess BLS certification.
• Demonstrate at least 1 year of relevant home health therapy experience.
• Strong knowledge of home health protocols and documentation procedures.
• Ability to work independently with minimal supervision.
• Exhibit excellent communication and time management skills.
