Job Summary:
A Physical Therapist is responsible for evaluating patients' posture, gait, balance, coordination, and strength to develop personalized treatment plans aimed at restoring mobility and independence. They assess functional limitations and implement therapeutic interventions to enhance patient outcomes.

Responsibilities:

• Assess patients’ physical conditions to identify impairments and functional limitations.
• Develop and implement individualized treatment plans based on patient evaluations.
• Guide and supervise therapy sessions, ensuring proper technique and patient safety.
• Document patient progress and adjust treatments accordingly to optimize recovery.
• Ensure compliance with all local, state, and federal healthcare regulations, standards, and facility policies.
• Collaborate with interdisciplinary team members to coordinate comprehensive patient care.

Qualifications:

• Hold a valid, unrestricted Physical Therapist license in the state of employment.
• Demonstrate a minimum of 1 year of prior PT experience, preferably in LTC or rehabilitation settings.
• Possess knowledge of skilled nursing home regulations, procedures, and guidelines.
• Maintain a valid driver’s license for independent travel if required.
• Submit a current work history/resume, references, license verification, and relevant certifications.
