Job Summary:

Provide inpatient physical therapy services to adult patients in a hospital-based rehab unit. Collaborate with healthcare team to develop and implement personalized rehabilitation plans, ensuring optimal patient outcomes.

Responsibilities:

• Evaluate patient needs and develop customized treatment plans based on clinical assessments.
• Administer therapeutic interventions to improve mobility, strength, and functional independence.
• Document patient progress accurately in the Epic electronic charting system.
• Maintain compliance with facility policies, safety standards, and certification requirements.
• Participate in patient education and discharge planning to ensure continuity of care.
• Adhere to scheduled shifts and available to work one weekend per month as required.

Qualifications:

• Hold a valid SC or Multi-State Compact Physical Therapist License at the start of the contract.
• Possess at least one year of clinical experience in inpatient physical therapy.
• Demonstrate BLS certification and ongoing compliance with hospital policies.
• Be proficient in Epic charting and electronic medical records.
• Ability to work independently and as part of a multidisciplinary team.
• Provide proof of licensure application if license is pending.
