Job Summary:
Provide compassionate hospice nursing care to patients in their homes and care facilities, delivering personalized support that enhances quality of life. Act as a key member of the interdisciplinary team to meet patients’ physical, emotional, and spiritual needs.

Responsibilities:

• Assess patients’ symptoms and develop individualized care plans to manage pain and other comfort measures.
• Educate patients and caregivers about disease progression, symptom management, and care strategies.
• Collaborate with the interdisciplinary team to coordinate patient services and ensure holistic care delivery.
• Lead and support patients in identifying their personal goals of care, fostering independence and dignity.
• Document patient progress, interventions, and outcomes accurately in electronic health records.
• Provide backup coverage for weekends and on-call duties to ensure continuous patient support.

Qualifications:

• Valid registered nurse (RN) license in good standing.
• Minimum of 2 years of clinical nursing experience, preferably in hospice, palliative, or chronic disease management.
• Strong assessment skills with the ability to think critically and respond effectively to changing patient needs.
• Excellent communication and teaching abilities to support patients, families, and caregivers.
• Ability to work independently with flexible scheduling to balance work and personal life.
• Basic proficiency with electronic medical records and documentation.
