Job Summary:

Provide compassionate hospice nursing care to patients in various home and facility settings, supporting their comfort and well-being while guiding families through the caregiving process. This role emphasizes personalized care, education, and symptom management to enhance quality of life.

Responsibilities:

• Assess patient conditions and develop individualized care plans to meet their goals and needs.
• Educate patients and caregivers on symptom management, medication administration, and self-care techniques.
• Collaborate with interdisciplinary teams to deliver comprehensive hospice services.
• Lead and coordinate care, ensuring smooth communication among team members and families.
• Provide emotional support and comfort to patients and their loved ones during difficult times.
• Document patient care activities accurately and maintain compliance with regulatory standards.

Qualifications:

• Hold a valid Registered Nurse (RN) license in the relevant state.
• Demonstrate clinical expertise in symptom management and patient assessment.
• Exhibit strong critical thinking and decision-making skills.
• Possess excellent communication and interpersonal abilities.
• Have prior experience in hospice, palliative, or home health nursing preferred.
• Maintain current CPR certification.
