Job Summary:

Provide specialized physical therapy services to patients in a home health setting, focusing on improving mobility and overall function through personalized treatment plans.

Responsibilities:

• Assess patient needs and develop individualized therapy plans.
• Administer physical therapy treatments in accordance with established protocols.
• Monitor patient progress and modify treatment plans as necessary.
• Maintain detailed documentation of patient encounters and treatment outcomes.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Comply with all home health regulations and documentation requirements.

Qualifications:

• Hold a valid license as a Physical Therapist.
• Demonstrate experience in home health care and familiarity with Home Base and Pointcare systems.
• Possess at least 1 year of home health experience.
• Exhibit strong assessment and communication skills.
• Be able to work independently within a home setting.
