Job Summary:
Provide comprehensive physical therapy services within a home health setting, focusing on patient evaluations, treatment plans, and follow-up care. Must be adaptable to travel extensively and maintain documentation standards.

Responsibilities:

• Perform start of care assessments and follow-up visits for adult patients in a home health environment.
• Develop and implement individualized treatment plans based on patient needs and clinical guidelines.
• Document all patient interactions accurately within the required timeframe using PointCare/HCHB EMR systems.
• Travel up to 75 miles between patient homes, ensuring timely and efficient visits.
• Participate in periodic office meetings and maintain compliance with on-call and documentation policies.
• Collaborate with healthcare team members to coordinate patient care and ensure quality outcomes.

Qualifications:

• Hold a current, valid physical therapy license in the state of practice.
• Possess at least 1 year of recent adult home health experience.
• Proficient in OASIS, HomeCare HomeBase, and HCHB EMR systems.
• Have a reliable vehicle, valid driver’s license, and agency-required liability insurance.
• Capable of traveling up to 75 miles from the assigned clinic location.
• Demonstrate strong documentation and clinical skills in a home health setting.
