Job Summary:

Provide skilled occupational therapy services to patients in a home health setting, assisting with recovery and improving daily functional abilities. The therapist will work closely with patients and care teams to develop personalized treatment plans.

Responsibilities:

• Assess patients' functional status and develop tailored therapy plans.
• Deliver occupational therapy interventions to promote recovery and independence.
• Document patient progress and maintain accurate records in compliance with clinical standards.
• Collaborate with healthcare teams to coordinate care and optimize treatment outcomes.
• Comply with licensing, documentation, and travel requirements.
• Participate in on-call rotations and maintain timely communication with team members.

Qualifications:

• Hold an active California occupational therapist license or be in the process of obtaining one.
• Possess at least 1 year of home health therapy experience.
• Demonstrate proficiency with OASIS documentation.
• Certification in BLS (AHA) is required.
• Have reliable personal transportation and a valid driver’s license.
• Be able to work Monday through Friday with flexibility for on-call duties.
