Job Summary:

Provide comprehensive physical therapy services to patients in a home health setting, ensuring personalized treatment plans and optimal outcomes. Deliver therapy with a focus on functional improvement and safety while adhering to all clinical and safety protocols.

Responsibilities:

• Evaluate patient condition and develop individualized treatment plans based on assessments.
• Administer therapeutic interventions to improve mobility, strength, and overall function.
• Document patient progress and update care plans in compliance with regulatory standards.
• Educate patients and their families on exercises, safety, and home programs.
• Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes.
• Maintain licensed certification and stay current with best practices and industry standards.

Qualifications:

• Hold a valid Doctor of Physical Therapy (DPT) license in applicable states or foreign equivalent.
• Possess current CPR or BLS certification from an approved vendor.
• Demonstrate possession of a valid driver’s license, with a minimum age of 21.
• Show ability to work independently and provide patient-centered care.
• Maintain compliance with state licensing and certification requirements.
