Job Summary:

Provides skilled rehabilitative care to patients with physical functioning disorders under the supervision of a Physical Therapist, utilizing evidence-based practices to support treatment plans and optimize patient outcomes.

Responsibilities:

• Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, and use of supportive devices.
• Assess patient progress and communicate with supervising Physical Therapist to modify treatments accordingly.
• Document all clinical activities, patient progress, and regulatory compliance in the electronic medical record system.
• Provide education, counseling, and support to patients, families, and caregivers regarding physical health and treatment goals.
• Collaborate with the healthcare team to develop discharge plans and treatment strategies.
• Support student supervision and assist with obtaining necessary supplies and DME as directed.

Qualifications:

• Hold an active State Licensure as a Physical Therapist Assistant.
• Possess at least 1 year of PTA experience, with LTC or rehab experience preferred.
• Demonstrate proficiency in clinical documentation and patient care procedures.
• Able to work effectively within a multidisciplinary healthcare team.
• Maintain adherence to APTA standards, code of conduct, and ethical practice.
• Exhibit strong communication and assessment skills.
