Job Summary:
Responsible for evaluating, diagnosing, and treating patients with communication, cognitive, or swallowing disorders. Develops personalized care plans and utilizes evidence-based practices to enhance patient outcomes while ensuring compliance with all regulatory standards.

Responsibilities:

• Assess and diagnose patients with speech, language, cognitive, or swallowing disorders using standardized assessments.
• Develop and implement individualized treatment plans based on clinical evaluations and evidence-based practices.
• Monitor patient progress and modify therapy interventions accordingly, documenting all updates in the medical record.
• Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members regarding speech and language disorders.
• Communicate regularly with supervisors and healthcare team members about patient progress, barriers, and treatment goals.
• Ensure accurate documentation of treatment sessions, including minutes and clinical notes, in electronic health records.

Qualifications:

• Hold active State licensure as a Speech-Language Pathologist.
• Certification from the American Speech-Language-Hearing Association (CCC-SLP) required.
• Minimum of one year of SLP experience, preferably in LTC or rehabilitation settings.
• Valid Texas license and BLS certification preferred.
• Demonstrate adherence to ASHA guidelines, state practice acts, and ethical standards.
• Ability to supervise and support CFY or student SLPs as needed.
