Job Summary:
Provide specialized physical therapy services to patients in a home health setting within Butte, Yuba, and Sutter Counties. Ensure compliance with all licensing and certification requirements while delivering quality care.

Responsibilities:

• Assess patients' physical conditions and develop personalized treatment plans.
• Implement therapeutic exercises and manual therapy techniques to promote recovery.
• Monitor patient progress and adjust treatment strategies accordingly.
• Document patient interactions and treatment outcomes accurately in compliance with agency standards.
• Educate patients and caregivers on post-treatment care and injury prevention.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Qualifications:

• Hold a valid License in Physical Therapy (DPT required if obtained after 2016).
• Possess at least 1 year of home health experience, or willingness to train.
• Maintain current BLS certification.
• Be at least 21 years of age.
• Have a valid driver’s license and reliable transportation.
• Show strong communication and interpersonal skills.
