Job Summary:
We are seeking a dedicated Physical Therapist to provide home health services through patient visits and assessments. The therapist will work independently, traveling to patient homes to deliver quality care and maintain accurate documentation.

Responsibilities:

• Evaluate patient needs and develop individualized treatment plans.
• Conduct home visits for start of care and follow-up assessments.
• Document patient progress accurately and timely using EMR systems.
• Travel up to 75 miles from the home clinic to reach patients.
• Collaborate with care teams during periodic office meetings.
• Ensure compliance with agency and regulatory requirements.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Have at least 1 year of recent adult home health experience.
• Proficient in OASIS documentation and HomeCare HomeBase EMR.
• Possess a valid driver’s license, reliable transportation, and agency-required liability insurance.
• Demonstrate excellent assessment and communication skills.
• Willing to travel and accept a flexible schedule.
