Job Summary:
The Physical Therapist is responsible for assessing and implementing individualized treatment plans to restore and improve patient mobility and function following injury or illness. They work within a home health setting to support patients' recovery and independence, ensuring optimal clinical outcomes.

Responsibilities:

• Evaluate patients' physical condition and develop personalized treatment plans based on medical prescriptions.
• Administer therapeutic interventions to improve mobility, strength, and overall functional ability.
• Document clinical observations, progress notes, and discharge plans accurately and timely.
• Supervise and collaborate with Physical Therapy Assistants as needed.
• Complete initial evaluations and notify the Clinical Team Manager of OASIS scores within established deadlines.
• Educate patients and caregivers on therapeutic exercises and home care strategies.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Possess at least one year of recent Home Health experience.
• Demonstrate Strong clinical assessment and treatment skills.
• Exhibit excellent communication and documentation abilities.
• Ability to work independently within a home health environment.
