Job Summary:
Provide specialized physical therapy services to patients in a home health setting, ensuring recovery and functional improvement through tailored treatment plans. This role requires a licensed clinical professional committed to delivering excellent patient care.

Responsibilities:

• Assess patient needs and develop personalized treatment plans based on clinical evaluations.
• Implement therapeutic interventions to promote mobility, strength, and independence.
• Monitor patient progress and adjust treatments accordingly to achieve optimal outcomes.
• Document treatment sessions and maintain accurate patient records per regulatory standards.
• Collaborate with healthcare teams to coordinate care and ensure seamless service delivery.

Qualifications:

• Hold a Doctor of Physical Therapy (DPT) degree; licensed in applicable states or foreign equivalent.
• Possess current physical therapy licensure in all states of practice.
• Have home health experience and familiarity with outpatient or community-based therapies.
• Maintain AHA BLS certification.
• Be able to provide DOB, SSN, RTO, and preferred start date upon submission.
