Job Summary:
Provide specialized physical therapy services to patients requiring rehabilitation and recovery support. Collaborate with healthcare teams to develop and implement individualized treatment plans that promote optimal functional outcomes.

Responsibilities:

• Assess patients’ physical condition and establish appropriate therapy goals.
• Design and execute personalized treatment plans based on patient needs.
• Monitor patient progress and adjust therapies accordingly.
• Document treatment sessions and maintain accurate patient records.
• Educate patients and families on exercises and care techniques.
• Collaborate with healthcare staff to ensure comprehensive care delivery.

Qualifications:

• Hold a valid state licensure as a licensed Physical Therapist.
• Possess proven experience in a skilled nursing or rehabilitation setting.
• Demonstrate strong assessment and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Maintain current CPR and basic life support certifications.
