Job Summary:
Provide specialized physical therapy services to patients in a skilled nursing facility, supporting recovery and improving overall mobility and function.

Responsibilities:

• Assess patients’ physical conditions and develop individualized treatment plans.
• Administer therapy sessions following prescribed protocols.
• Monitor patient progress and adjust therapies accordingly.
• Collaborate with healthcare team to ensure cohesive patient care.
• Document patient assessments, treatment plans, and progress accurately.
• Educate patients and their families on post-treatment exercises and care.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess a degree from an accredited physical therapy program.
• Demonstrate experience working in a skilled nursing or rehabilitation setting.
• Exhibit strong interpersonal and communication skills.
• Maintain current CPR and BLS certifications.
