Job Summary:
Seeking a licensed Physical Therapist to provide patient-centered rehabilitation services in a skilled nursing facility. The therapist will assess, plan, and implement therapy programs to promote functional recovery and improve patient outcomes.

Responsibilities:

• Evaluate patients' condition and develop individualized treatment plans.
• Administer therapy sessions focused on restoring mobility, strength, and range of motion.
• Collaborate with interdisciplinary teams to ensure optimal patient care.
• Maintain accurate documentation of treatments, progress, and discharge planning.
• Adhere to facility policies, safety standards, and regulatory guidelines.

Qualifications:

• Valid state license as a Physical Therapist.
• Proven experience working in a Skilled Nursing Facility or similar post-acute setting.
• Strong knowledge of therapeutic techniques and rehabilitative procedures.
• Excellent communication and interpersonal skills.
• Ability to work flexible hours including weekdays with weekend rotation.
