Job Summary:

Provide specialized occupational therapy services to patients in a skilled nursing facility setting, focusing on enhancing functional independence and quality of life through tailored therapeutic interventions.

Responsibilities:

• Evaluate patients' physical, cognitive, and emotional conditions to develop individualized treatment plans.
• Administer modalities, ADL training, and fall and balance assessments to promote rehabilitation.
• Implement programs for dysphagia management, including Vital Stim therapy.
• Document patient progress accurately using Net Health E-Doc and IPads.
• Collaborate with a cohesive rehab team to optimize patient outcomes and maintain regulatory compliance.
• Educate patients and families on therapy goals, procedures, and home programs.

Qualifications:

• Hold a valid occupational therapy license.
• Possess experience in skilled nursing or long-term care environments.
• Demonstrate proficiency in modalities, dysphagia, and dementia programs.
• Ability to utilize electronic documentation tools effectively.
• Exhibit strong communication and team collaboration skills.
