Responsibilities:

• Assess patients' physical, emotional, and cognitive conditions to develop personalized treatment strategies.
• Implement therapy plans to improve patients' daily living skills and physical functions.
• Document patient progress accurately and communicate with the healthcare team.
• Adapt therapy sessions based on patient response and evolving needs.
• Educate patients and caregivers on treatment goals and home exercise programs.

Qualifications:

• Hold a valid Occupational Therapist license and state registration.
• Possess proven experience in a skilled nursing or long-term care environment.
