Job Summary:
Provides skilled nursing care to patients in their homes, working collaboratively with the interdisciplinary team to promote health, safety, and independence. Offers a balance of autonomy and team support, ensuring compassionate patient-centered care.

Responsibilities:

• Deliver nursing services to individuals and families in their residences under RN supervision.
• Perform prescribed patient care activities based on personalized care plans.
• Assist RNs in developing, evaluating, and updating patient care plans as needed.
• Observe, document, and report on patients’ physical and emotional conditions, including reactions to care.

Qualifications:

• Hold an active Licensed Practical Nurse license in Vermont or New Hampshire, or an unencumbered compact license.
• Graduate from an accredited practical nursing program.
• Possess at least 2 years of experience in a home health setting.
• Demonstrate strong assessment and communication skills.
