Job Summary:
The RN Case Manager in Home Health provides personalized nursing care to patients in their homes, coordinating treatment plans and ensuring quality outcomes. This role emphasizes patient-centered care, interdisciplinary communication, and promoting independence for patients and caregivers.

Responsibilities:

• Develop and periodically review comprehensive care plans in collaboration with physicians, patients, and families.
• Assess and monitor patients’ clinical, functional, and emotional status to identify needs and progress.
• Coordinate care activities across multidisciplinary teams and communicate updates effectively.
• Educate patients and caregivers on self-care techniques, medication management, and health maintenance.
• Document evaluations, care plans, treatment goals, and significant clinical changes accurately.
• Advise on patient discharges and ensure seamless transition of care.

Qualifications:

• Hold an active, unencumbered RN license in Vermont and New Hampshire or a valid compact license.
• Graduate from an accredited nursing program.
• Possess a minimum of 2 years of experience in Home Health settings.
• Demonstrate strong clinical assessment and communication skills.
• Ability to work independently while contributing to a team environment.
