Job Summary:

The Registered Nurse in Home Health provides comprehensive nursing care to patients in their homes, following physician orders and agency protocols. They coordinate patient care, supervise home health aides, and ensure documentation accuracy to support quality outcomes.

Responsibilities:

• Assess patient health needs and develop individualized care plans.
• Administer medications, treatments, and wound care as prescribed.
• Supervise and evaluate Home Health Aides and LPNs/VNs.
• Document patient progress and care activities accurately within the required timeframe.
• Travel to patient homes and attend periodic office meetings.
• Collaborate with physicians and healthcare teams to optimize patient outcomes.

Qualifications:

• Hold an active RN license in the state of practice.
• Possess at least 1 year of recent Adult Home Health or Hospice experience.
• Proficient with HomeCare HomeBase (HCHB) EMR system within the last 6 months.
• Must have reliable transportation, valid driver’s license, and auto insurance.
• Demonstrate excellent clinical assessment, communication, and documentation skills.
