Job Summary:
Seeking an experienced Registered Nurse for a Home Health assessment role. The ideal candidate will conduct comprehensive evaluations of patients in their homes, ensuring quality care and compliance with regulatory standards.

Responsibilities:

• Perform initial in-home health assessments, evaluating patient health status, strengths, and care needs.
• Review medical documentation and referral forms to inform evaluation.
• Document evaluation findings accurately in electronic systems and transmit data as required.
• Communicate effectively with consumers regarding care options, supports, and service vendor choices.
• Collaborate with interdisciplinary care teams to enhance care coordination and service delivery.
• Identify opportunities for health promotion, illness prevention, and resource support.

Qualifications:

• Hold current RN licensure in New York State.
• Possess a minimum of two years’ recent experience in a home setting, including assessments similar to UAS or OASIS.
• Demonstrate experience caring for geriatric populations and managing chronic diseases at home.
• Show proficiency with technology and knowledge of state and agency regulations governing nursing.
• Ability to work independently in a fast-paced, deadline-driven environment with strong communication skills.
• Prefer candidates with UAS-NY assessment experience (role 40 or 45) within the last year.
