Job Summary:

The RN Nurse Assessor is responsible for conducting comprehensive home assessments to evaluate patients' health status, needs, and preferences. This role involves coordinating care plans, reviewing medical documentation, and collaborating with interdisciplinary teams to ensure seamless patient care delivery.

Responsibilities:

• Perform initial in-home evaluations to assess health conditions, strengths, and care requirements for long-term planning.
• Review medical records and referral documentation to support assessment accuracy.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns or discrepancies identified during assessments to ensure quality and continuity of care.
• Discuss healthcare options, supports, and service vendors with consumers to facilitate informed choices.
• Communicate with the Call Center and eligible consumers for managed care enrollment.
• Collaborate with care team members to enhance care coordination and service delivery.
• Collect quality review data and maintain case notes within CRM platforms.
• Identify opportunities for health promotion and disease prevention in the home setting.
• Stay informed on community resources, payor requirements, and network services relevant to the target population.
• Maintain compliance with all regulatory and agency standards.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two years recent experience conducting home assessments within the last ten years.
• Experience working in settings such as CHHA, LHCSA, MLTC, or similar.
• Proven ability to perform assessments similar to UAS or OASIS.
• Strong communication skills, both written and verbal.
• Ability to work independently in a fast-paced environment.
• Demonstrated proficiency with electronic health records and assessment tools.
