Job Summary:

Responsible for performing comprehensive patient assessments, developing individualized care plans, and coordinating services in a home health setting. Ensures high-quality patient care through effective evaluation and collaboration with interdisciplinary teams.

Responsibilities:

• Conduct initial in-home evaluations to assess health status, care needs, and patient preferences.
• Review medical documentation and relevant health referral forms for accuracy and completeness.
• Document evaluation findings accurately and transmit data electronically as required.
• Communicate any concerns or conflicting information identified during assessments.
• Collaborate with consumers and care teams to discuss healthcare options and support services.
• Maintain accurate case notes, collect quality review data, and support outcome measurements.

Qualifications:

• Hold current RN licensure in New York State.
• Possess at least two years of recent experience in a home health setting within the last ten years.
• Demonstrate experience conducting assessments similar to UAS or OASIS.
• Have home care experience caring for geriatric populations and managing chronic diseases.
• Ability to work independently and adapt to a fast-paced, deadline-driven environment.
