Job Summary:
The RN - Nurse Assessor conducts comprehensive in-home evaluations to assess patients' health status, care needs, and preferences. The role involves reviewing medical documentation, documenting findings, and facilitating care coordination within a fast-paced environment.

Responsibilities:

• Perform initial in-home assessments to evaluate health status, strengths, and care needs of patients.
• Review medical documentation and referral forms relevant to each case.
• Document evaluation data accurately in electronic systems and transmit as required.
• Communicate effectively with consumers regarding health care options, support services, and waiver programs.
• Collaborate with interdisciplinary care teams to improve care coordination and service delivery.
• Collect and record quality review data to support outcome measurements and record case notes into CRM.

Qualifications:

• Current RN licensure in New York State.
• At least two (2) years of recent experience conducting home-based assessments within the last ten (10) years.
• Proven experience in home care settings such as CHHA, LHCSA, or MLTC, including assessments similar to UAS or OASIS.
• Strong knowledge of state regulations, community resources, and payor requirements.
• Excellent organizational, communication, and interpersonal skills.
• Ability to work independently in a deadline-driven environment and manage complex tasks.
