Job Summary:

Provide comprehensive hospice nursing care to patients in their homes, ensuring comfort, symptom management, and emotional support. Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet patients' unique needs.

Responsibilities:

• Perform patient assessments to evaluate appropriateness for hospice care and progress toward established goals.
• Develop, implement, and evaluate personalized care plans based on evidence-based practices.
• Educate patients and families using age-appropriate methods, documenting teaching tools and revisions.
• Maintain accurate documentation, including admission, re-visit, and discharge assessments within required timeframes.
• Participate in interdisciplinary team meetings biweekly to coordinate care and revise plans as needed.
• Ensure medication reconciliation, monitor safe medication management, and adhere to infection control protocols.

Qualifications:

• Minimum of two (2) years of RN experience in a home hospice setting.
• Valid New Jersey RN license and current BLS certification.
• Experience with EMR systems, strong critical thinking, and excellent communication skills.
• Valid New Jersey driver’s license.
