Job Summary:

Provides comprehensive hospice nursing care to patients in their homes, ensuring comfort, safety, and quality of life. Collaborates with interdisciplinary teams to develop and implement individualized care plans.

Responsibilities:

• Assess patients' eligibility for hospice services and evaluate learning needs and progress toward goals.
• Develop, implement, and revise plans of care based on clinical evidence and patient needs.
• Perform timely admission, re-visit, and discharge assessments, ensuring accurate documentation.
• Educate patients and families using age-appropriate methods; revise teaching plans as needed.
• Supervise Home Health Aides, ensure appropriate service utilization, and communicate progress with the Interdisciplinary Team (IDT).
• Maintain compliance with hospice regulations, insurance guidelines, and infection control protocols; participate in IDT meetings biweekly.

Qualifications:

• Hold a current New Jersey RN license with at least two years of home hospice experience.
• Demonstrate proficiency with EMR systems, strong critical thinking, and effective communication skills.
• Possess and maintain valid New Jersey driver’s license and BLS certification.
• Experience in coordinating care and documenting activities within a home health environment.
