Job Summary:
The Licensed Practical Nurse (LPN) in LTC/SNF provides direct patient care, administers medications, and collaborates with healthcare team members to ensure resident well-being. They monitor residents' conditions, communicate changes, and support nursing staff in daily operations.

Responsibilities:

• Administer medications to residents following the 5 Rights of medication administration.
• Report suspected abuse, neglect, or misappropriation to appropriate authorities.
• Notify charge nurses and physicians of any changes in residents' conditions.
• Assist CNAs with answering call lights and other care responsibilities.
• Update family members and healthcare providers regarding resident status.
• Perform additional nursing duties as requested by the nursing leadership.

Qualifications:

• Hold and maintain an active Wisconsin LPN license.
• Maintain current CPR/BLS certification.
• Possess at least 1 year of related healthcare experience preferred.
• Demonstrate exceptional accuracy and attention to detail in medication handling and documentation.
• Show strong communication skills with residents, families, and staff.
