Job Summary:
Provides specialized physical therapy services to patients with physical functioning disorders and wounds, focusing on assessment, treatment, and rehabilitation. Ensures compliance with regulatory standards while delivering evidence-based care to improve patient outcomes.

Responsibilities:

• Screen, evaluate, and develop personalized care plans for patients with physical and wound care needs.
• Administer various therapeutic procedures including manual techniques, ambulation, exercises, modalities, and use of assistive devices.
• Monitor patient progress and adjust treatments accordingly, documenting all clinical activities in electronic health records.
• Assess treatment outcomes, modify services as needed, and refer patients for additional support if required.
• Educate and counsel patients, families, and caregivers on treatment processes and recovery strategies.
• Communicate effectively with team members regarding patient progress, barriers, and treatment plans.

Qualifications:

• Hold an active Massachusetts State License as a Physical Therapist.
• Possess at least 1 year of PT experience; new grads with Level II experience are eligible.
• Demonstrate experience in long-term care or rehabilitation settings.
• Be proficient in clinical assessments, documentation, and regulatory compliance.
• Ability to supervise and support therapists, aides, and students.
• Adhere to APTA guidelines, professional conduct, and ethical standards.
