Job Summary:

Provides evaluate, develop, and implement individualized treatment plans for patients with physical functioning disorders, including wound care. Utilizes evidence-based practices to improve patient outcomes and enhance quality of life.

Responsibilities:

• Screen and assess patients to determine their physical therapy needs and develop appropriate care plans.
• Administer therapeutic procedures such as manual techniques, ambulation, exercises, modalities, supportive devices, and wound care.
• Monitor patient progress, adjust treatments, and document outcomes in compliance with regulatory and clinical standards.
• Educate patients, families, and caregivers on treatment plans, prevention strategies, and home exercises.
• Collaborate with healthcare team members to coordinate patient care and ensure optimal recovery.
• Complete discharge summaries and maintain accurate, timely documentation of services provided.

Qualifications:

• Hold a valid MA State Physical Therapist license.
• Demonstrate at least 1 year of clinical PT experience, preferably in LTC or rehab settings.
• Possess experience with standardized assessments and evidence-based interventions.
• Be able to supervise PT assistants, aides, and students as needed.
• Exhibit strong communication skills and adherence to ethical standards governed by APTA and state regulations.
