Job Summary:
Provide specialized occupational therapy services to patients in home health settings, focusing on improving functional independence and quality of life. Ensure compliance with regulatory standards and tailor treatment plans to individual patient needs.

Responsibilities:

• Evaluate patients' functional status and develop personalized treatment plans.
• Administer therapeutic interventions to promote rehabilitation and recovery.
• Document patient progress and adjust therapy plans accordingly.
• Collaborate with healthcare teams to coordinate care and ensure optimal outcomes.
• Maintain accurate records in the EPIC EHR system.
• Adhere to safety protocols and ensure compliance with licensure and certification requirements.

Qualifications:

• Hold a Master's degree or Doctor of Occupational Therapy (OTD); license must be current and obtained on or after January 2007 or equivalent from abroad.
• Possess valid occupational therapy licensure in the state of practice.
• Have a minimum of 2 years of recent clinical experience in the specialty within the last 3 months.
• Demonstrate proficiency with EPIC electronic health records.
• Maintain current BLS certification.
