Job Summary:
Seeking a qualified Outpatient Physical Therapist to provide patient care in a hospital-based outpatient rehab setting. The therapist will assess, develop, and implement treatment plans to improve patient mobility and function, ensuring quality outcomes.

Responsibilities:

• Evaluate patients' conditions through physical assessments and develop individualized treatment plans.
• Implement therapeutic interventions utilizing evidence-based practices to achieve functional goals.
• Collaborate with healthcare team members to coordinate patient care and progress.
• Document patient progress accurately in the electronic health record system.
• Adhere to facility policies, safety standards, and state licensing requirements.
• Float within scope to other units or facilities as needed to meet operational demands.

Qualifications:

• Hold a valid South Carolina or Multi-State Compact Physical Therapist License.
• Possess at least 1 year of outpatient therapy experience.
• Be BLS certified.
• Demonstrate proficiency in using electronic charting systems (e.g., Epic).
• Have successful compliance with travel and licensing requirements.
• Able to work 8:00am to 4:30pm shifts on weekdays.
