Job Summary:
Provides direct patient care to residents in long-term care or skilled nursing facilities, supporting the nursing team in delivering quality healthcare services. Responsible for assisting with daily activities, monitoring patient conditions, and ensuring comfort and safety.

Responsibilities:

• Assist patients with activities of daily living such as bathing, dressing, feeding, and personal hygiene.
• Monitor and document vital signs, patient symptoms, and responses to treatments accurately.
• Follow care plans and report any changes in patient conditions to the nursing staff.
• Maintain infection control standards and adhere to universal precautions.
• Support medication reminders and assist with restorative care activities, including mobility exercises and use of assistive devices.

Qualifications:

• Demonstrate at least 6 months of experience in a long-term care or skilled nursing facility.
• Possess current certification in Basic Life Support (BLS) for Healthcare Workers.
• Ability to communicate effectively in English, both verbally and in writing.
• Show patience, empathy, and respect for patients with diverse needs and backgrounds.
• Maintain emotional stability and professionalism under physical and emotional pressures.
