Job Summary:
Seeking a compassionate and skilled Registered Nurse for a Long Term Care/SNF setting. The ideal candidate will provide high-quality patient care, administer treatments, and coordinate with healthcare teams to ensure optimal patient outcomes.

Responsibilities:

• Perform patient assessments and develop personalized care plans.
• Administer medications and treatments in accordance with physician orders.
• Communicate pertinent patient information to MD/NP and document accurately.
• Assist patients with activities of daily living and provide wound care as needed.
• Monitor and operate medical devices such as gastrostomy tubes, urinary catheters, wound vacs, CPAP, and tracheostomies.
• Ensure proper patient admission and discharge processes; maintain compliance with facility protocols.

Qualifications:

• Active Hawaii RN license required prior to start date.
• Minimum of one year of experience in Long Term Care or SNF settings preferred.
• Proficient in patient assessments and clinical procedures.
• Able to handle a flexible schedule with varied shifts, including weekends and holidays.
• Strong communication and documentation skills.
• BLS certification required; additional certifications a plus.
